Decision records

A step of the Claude Code development environment tree on Gradus, in the Project memory area: CLAUDE.md, docs, conventions.

Record each non-obvious decision as a short dated note with the alternatives rejected, so later sessions do not relitigate it.

Usually comes after: Knowledge base in docs.

Add a docs/decisions/ directory with a numbered Markdown template (context, decision, alternatives considered, consequences). Write the first record for the most important decision already baked into the codebase and link the directory from docs/index.md.
